A freight train can transport goods more immediately, and with lower costs. Transporting goods over seas requires, well, one or more ships. And a port on both the starting point and the destination. Both of those things require much greater costs than transport over land.

how would a single freight train route change the position of Sicily so drastically?
It wouldn't make sense to ship cargo to Sicily when you have ports elsewhere in Italy that can be attached with vastly larger freight routes.
