Look up Retention Holsters. Most concealed carry holsters are level one holsters. Same goes for law enforcement.
Still possible, just takes effort, strength and requires a bit of a struggle.

All good open carry holsters, like those used by police and military have a locking mechanism (active retention) with a lever located on the inner side of the holster, so if you wanted to steal the gun you would need to get your fingers in between the gun and the man's body.
