but in this case it was clearly the man in charge of the reactor. He obviously had no idea what he was doing, he never bothered consulting the people who actually build it or knew more about it than he did, he didn't do research into what could happen. his bullshitting made it worse for everyone.

The design FLAWS were a large contributor TO the explosion. That would be part of the "other entities at fault" I menionted. For example, the control rods were far too short. They used a dangerously large void coefficient (something NO OTHER REACTOR ANYWHERE does) - this causes the reactor to be unstable at low power levels. This was not something known to any of the crew operating the reactors. it was ONLY known to the designers of the reactor.

Chernobyl was operational for many years after the accident and the other RBMKs had no issues as they were operated by the book.

also developing additional safety measures, testing them, producing them and installing them takes some time so 2 years is a good timeframe for an operation of such magnitude.

Ukranians were the ones to ask for help of European Companies. Soviet Union contained the disaster itself. The liquidators were given no more than 90 seconds of exposed radiation, and it was pure voluntary basis (in total, about 3500 volunteers showed up across the country).
