The universe will continue until its climax, which is either an eternity of photons in a state of absolute zero, or a reset of sorts.
Assuming the first one, then even if consciousness is eternal then nothing awaits any of us.
Assuming the second one, then at the moment of death, whatever consciousness awaits you will be the very next thing you experience, and the transition will be instantanious.

actually the current leading theory is one of endless expansion, and thus maximum entropy being reached.
