So what the was problem with their doctrine that allowed an aircraft to be shot down like that? What mistakes were made? There must be thousands of RPGs in circulation in Afghanistan, and very rarely do aircraft get shot down with them, so what went wrong?

The Night Stalkers/SEAL's got a little two ballsy and tried inserting a SEAL team right into a active firefight with no Apache support or support at all. Thus one raghead was able to get a EZ shot on one of the chinooks

I imagine they were relatively slow and low looking for the SEALs on the ground and it was still broad daylight. The Taliban would have been well hidden abd had an easy target.

I thought there was apache support once the rescue mission was launched.

No. Only one Chinook went in initially and that was Turbine 33. The second one came in later with reinforcement and after CAS had cleared the area. Lutrell was long gone.

Maj. Reich made the call to go in ASAP and not wait for Apache support. That's why he took the stick.

Basically the OP was poorly put together and poorly executed. It was publicly covered up to make a feel good hero story, but behind the scenes most understand that it was a huge clusterfuck.
They were compromised early, and were a team outnumbered and outgunned. They made a number of mistakes like not properly stowing the fast ropes, and being easily tracked by the ACM. Additionally they allowed a laptop with classified intel like Embassy layouts in Kabul to be retrieved by the enemy.
