It's fucking worse when you haven't plugged your phone in a place with wifi so it tries to auto update a shitton of worthless system apps while you're on ANY wifi even if you're not plugged in.
I spent like two weeks without wifi in the hotel while on vacation and by the end of it the thing was so desperate to update it even started updates as I walked by the lobby with the free public wifi.

AFAIK that's what happens when an app changes its icon, happened to me a few times. App updated at night an in the morning my homescreen had an empty spot. Had to open the launcher and put it back there, maybe it's the same for you.
