MITM attacker modifying the NTP packet to expose a client-side vulnerability. Depending on the vulnerability this can be really bad, as ntp usually needs root privileges to set the time.

Time is often used with encryption protocols.
Change your time to hours in the past or future and watch SSL stop working.
If an attacker works out what key you will get at a certain time, they could exploit NTP to change your system time in order to force a key that'll allow them to read your traffic.

I use the NIST time servers, so they'd need to change the NIST servers, or somehow manage to MITM spoof the server and send out the wrong time.

It's just not really a feasible attack for 99.99999% of cases.

You don't change the servers. The whole point is that NTP is not secure, so you could easily MITM it.

Explain it then.

I have to send a request to the NTP server, I send that request to a specific known address.

You would need to pretend to be that answering server by either spoofing the DNS registry or similar.

You can't just send a packet to my computer claiming to be an NTP server and my computer just says SURE WHY NOT.

Sending it to a "specific address" is as secure as plain http. As in not at all, it's not hard to mitm
