Literal power seems realistic to me - it's a very cost-efficient way of converting nutrients to power, with the downside that you have to provide entertainment to the brain or it stops working properly.

You can "exit" the illusion by refusing to participate, but to do so is to refuse to deviate from the social canon, which society doesn't tolerate. And trying to enact any kind of systemic change is useless in the long run because the phenomenon is emergent and historically recurs.
