In America we say "with all due respect", instead of "greatest respect". It's a more blunt and easily understood insult.
These statements on their own don't really mean anything. It's important to know the context they're being said in to actually make a judgement on which meaning is intended, and in context I think Americans would be able to tell more or less as well as Brits if there was an implicit meaning present or if the person actually meant what they said.
If you look at the results, you'll see there are a lot of Brits who, without context, would make the same default assumptions as Americans.
