Manizales is a small city of about 400k residents. That picture is very old, maybe form the 1990s. Here is a more recent one but saddly the church and snowy peak don't appear in the pic

The thing is that most people live somewhere in that region so that is why most cities are there. The remaining 25% of the population lives either in the Caribbean region, Pacific region or Llanos orientales.
The Amazon region, and basically the southeastern half of the country is very sparsely populated. Only like 1% of the total population lives there
