the weird thing is that until less than 100 years ago there was no new town,Taranto was basically just the island and 150-200k people gravitated around it. After urbanization in the 60s, 70s and 80s everyone moved out and now there's less than 4000 people living in Taranto vecchia. Most of those people are either fishermen that have their boats on the seaside of Mar Piccolo or drug dealers that enjoy the safety of all those intricate small roads to hide.
It's not even that sketchy, there's basically no organized crime in Taranto, just a lot of small time hustlers constantly doing minor crimes, its basically safe to walk around at any time of day or night.

for a good 2000 years the old town had somewhere between 40000 to 70000 but most of the commerce and life was there so the rest of the city would gather around it.

most of them yeah. In the past ten or so years a lot of upper middle class people started buying the more interesting ones where noblemen and landowners used to live to redo them but it's only in the central street (Via Duomo), the moment you step out of that road everything's abandoned for the most part.
