You know this isn't even the case in America, right? If you can't pay for healthcare yourself, you fall under the Affordable Healthcare Act. The difference between the Netherlands and America is that those who can pay for it have the option not to in America, while they're forced to take a (private) insurance in the Netherlands. I prefer the Dutch system because it leads to less mess in the long run (like the question whether a doctor should treat someone without insurance/money) while keeping government intervention to an acceptable minimum.
