They don't. This boy has permanently fucked his body up by taking HRT medication from a young age and basically trapping himself in the body of a teenage boy. Despite all of this he is like 6ft tall and examining the face you can tell it's a man anyway.

Cases of kids as young as 6-7 "transitioning", this is child abuse and will be recognised as such shortly. The parents often shop around for doctors until one agrees. It's really not going to end well for these people.
