When the big 2011 tsunami hit, the Japanese gov't set up a webpage in the wake of the disaster for people to upload identifying information of family members or anyone they suspect might have been in the disaster zone at the time, including a photo. If someone involved in the recovery effort knew that the person was dead, the web page would say so.
Someone added Ken to the page as a joke and people made up a back story that he announced he was traveling to Japan at the time. Eventually it turned out that the photo itself was just taken from some random ladys blog, and the copypasta was just a copypasta.
