They are tied up in too much bullshit to be forcefull with patients.
Alot of times they settle for far less than what the bill is worth, like maybe the patient will pay 2,000 dollars over the course of a year on a bill that was originally 10k or more.
Other times, a private company with contacts to the hospital will buy the debt for a fraction of the total cost, even less than if the patient had just paid.
They now own the rights to collect on that patient's debt, and unlike the hospital, they tend to be a bit more comfortable in breaking the law and harassing and lying to the debtor.

We don't work in third world factories. We have a service economy. Additionally a lot of the stocks and bonds (associated with banks and the 1%) are invested in by retirement funds, as, individuals, etc. so the whole debt issue is even misleading.
