Michigan had a couple proposals passed last year that will help to ungerrymander some of our voting districts, and I think Ohio is doing something similar in the future. As much as I hate zooms for not voting, it can pretty difficult given the state.

The price discrepancy is a result of pharmaceutical companies being allowed to charge whatever they want for a drug. The same issue occurs in hospitals, where they can also set their own prices for services.
Our government isn't allowed to do that even though it funds plenty of their medical research.
We have government price negotiation, kouhai. You can't get reasonable prices without the government coming to the table and making demands. It's a fundamental part of our public healthcare system.
The logic behind it is that competition will drive the prices down eventually, but that "eventually" can be a really long time with Amreican patent law. To a much lesser extent that problem exists in the Netherlands as well. A while ago there was a huge controversy over this one company demanding exhorbitant prices for a new heart medicine and one pharmacy decided to create its own version of it, sparking a huge debate about whether or not that pharmacy was acting within the confines of the law or not. Never heard the conclusion of that case so I guess it's still ongoing.

Medicaid does have negotiated prices. Even if it didn't, more Americans are covered by private insurance than by Medicaid. This huge market does help lower prices somewhat, but they lower them from whatever astronomical price the pharma companies decide to list a drug at.
Our government food and drug regulation agency isn't allowed to set the list price, but our government funded health insurance program is allowed to negotiate the prices they actually pay for drugs.

When Pfizer wants to put a new drug on the market, the FDA determines whether or not it is safe, but they aren't allowed to sit down with Pfizer to work out a reasonable price to sell the drug at. The process basically goes like this:

>4: company picks a price to list the drug at ($2000 in the case of Truvada)
>5: customer either pays $2000 for Truvada, or the customer's insurance provider (whether through a public or private program) negotiates down from the $2000 list price
Obviously the whole system is broken, but step 4 is the main reason our non-generic drugs are expensive.
