The Demon core accident was truly stupid. The people knew what they were playing with. There was a lot of ways it could have been avoided, even a simple wood structure would have prevented the total disaster.

Essentially an experimental small scale reactor that was supposed to be easy to put together and would work on arctic conditions.
They ran test runs with 3 man teams.
One of the teams managed to fuck up, by lifting the control rod that housed all the fuel beneath it in a chamber too high up, making the fuel cells go critical creating a massive amount of radioactive steam that launched the control rod straight up.
The rod impaled a dude observing the procedure in a catwalk over the other two to the ceiling. Rod entered through the groin and came out of his shoulder.
Other two got sprayed with a shitload of radioactive super heated steam, killing one of them instantly while the other was heavily injured.
Rescue team members had to switch in and out every 1 minute due to the exposure, and getting the surviving member out took like 2 hours. He was obviously so heavily dosed he died later in hospital
Basically at a navy reactor were three guys and it. Improper removal of a control rod, goes critical, partial meltdown which causes it too vaporize. The vaporized fuel pushes the cooling water upward with enough force to slam the whole reactor 27 feet airborne. The shield plugs explode outward and impale one guy to the reactor ceiling. The other two guys are hit with superheated radioactive steam. They believe one of the guys intentionally sabotaged it.
