What I would do if I didn't want to downloda any software (cause paranoid):

Open PDF (scan it first).
Take a picture of the entire page.
Put that shit into Paint or your replacement if you are macfag.
Write your text or scribble your sig.
Copy that image into word and/or export to PDF then and there.

Check and make sure it looks ok.

The default Mac PDF viewer automatically detects boxes and sections that require you to enter info, so it allows you to simply type everything
