Simple. The burden of healthcare is placed on employers and individuals
Instead of all citizens. Those that cannot afford health insurance are allowed to receive government funded healthcare. In my opinion, it’s fair because some people make poor lifestyle choices and we should not be burdened with those costs.

Also, nothing is free. The universal healthcare is paid by the State via the citizen's taxes and needless to say the State takes the opportunity to take a margin on it.
In short, you pay for it and don't necessarily needs it, and it is more expensive for the average citizen than if he paid the hospital directly by himself when he is sick, because of the State which plays the intermediate role between the citizen and the hospital and of course takes a very large margin for his politicians.
