Yeah its never as extreme as the movies but they do go thru extreme episodes that can be very scary. This guy was completely normal all last week and we all thought he was about to be released until last night plus he's really big and strong and we were frightened to physically confront him

Most don't even have extreme episodes it differs from person to person and lots of them won't have episodes.
I think the severity could have something to do with how long until it's detected and they start helping them because it never ever starts as something big like seeing things or hearing things or being delusional about your identity it's often starting as whispers or shadows or really small things you don't even notice always and if it's treated early it could help prevent it becoming serious, at least in some cases.
