The US doesn't have to pay if it can't at some point, they'll just take a credit hit, but then again we lead the global economy so that would fuck the world over, meaning no one got fucked relatively speaking. Debt is great when it provides leverage instead of just being free money for a politician to push a clientelist agenda. Debt service cost is the biggest downside and a continuous drag on a budget. Being foreign or domestic owned really makes no difference. The only thing that truly matters if it's denominated in a domestic or foreign currency, because the former allows for devaluation to get out of troubles.

Just google debt if you're that clueless. Who do they have to pay the money back to? It depends on the country. Most EU countries owe it to the EU and French/German banks, Japan owes most of its debt to itself.
Debt is not bad per se if you use that money correctly, imagine having borrowed 300k in 2015 and put it all on BTC for example; in that situation, debt was good. The problem here is that states are becoming addicted to debt, politicians fear population unrest if they cut public services or raise taxes so they just pay for public shit with debt. In some countries like Italy that debt is reaching the point where the interests are becoming bigger than the GDP annual growth thus absolutely stagnating the economy.
