How i've heard it work in the past was as simple as the hacker calling a carrier and pretending they worked for a store, i.e. best buy, and were with a customer who bought a new phone. The hacker would aound urgent like the customer was being an ass and the carrier would swap sims. I'm guessing the hacker would also need basic information about their target

There are ways around everything. Vendor verification won't stop a hacker. If anything, it would make it easier once a hacker has a verification number.
