you need a penis png, and then you can shape the mouth shape with the forward warp tool or use a donor mouth in the right shape. From there it's pretty much making transformations to make it fit right. BJF was particularly good at it, mine are not as good. pic related is more of his work

I believe so, as far as my knowledge goes I think he froze her in the foreground, obscured the background, and then used the perspective warp tool on her head as well as some transformation on her mouth edges to simulate a sucking motion. sorry, i dont really know the exact process. In pic related someone just used the perspective warp alone, and you can tell the difference between the two examples.

This user explained the basics pretty well but i'll add my two cents.
The animation can be done in photoshop as well. Click on Window >> Timeline to bring up the frame editing tool. Click on 'Create Timeline', press the button with the three squares to 'Convert to Frame'. Now you can see your first frame on the bottom left.
The background layer can stay the same through out the process. For the moving parts (girl and penis layers) you will need to create a new copy each time you make a new frame. Also you have to make sure that the layers used in a previous frame are not visible by clicking the eye icon next to the layer. Failing to do this will get your animation all messed up.
You will need to make at least 6-7 frames for the animation to be more or less fluid. When you're ready, select all frames and set the looping to 'Forever' and the 'frame delay' to 0.08s. You can fine tune the delay to get a smooth animation.
To save as an animated gif, go to File>Export>Save for Web
