What if the Holy Roman Empire at it's territorial apex suddenly reformed today in 2018? What would a reformation of an age old realm like this change? What would come of this? What would the political implications and ramifications of something like this be?

It would be a pile of shit, the HRE's big problem was the same as the EU's, it's too big for it's own good.

Not to mention Sicily, southern Italy, and the entire Holy Land (Frederick 2, the Stupor Mundi). Everyone apparently forgets that he fucked everyone up by giving power to the princes, self-rule to Sicily, and a state to the Tectonic Order, as long as they remained members of the Empire.
