Blue eyes were already there in darker skinned WHG, at least the now common mutation in the HERC2 locus. Either genetic bottleneck made it ubiquitous or selection for the mutation as HERC2 is involved in brain development and the actual pigment gene OCA2, its neighbor is only shut down as a side effect.

Depends on the size of source populations. Many genetic diseases are either due to founder's effect or 'adaption with a price attached' (see sickle cell anemia or mucoviszidosis which if heterozygous seems to protect against diarrhea). Else beyond a certain threshold of people inbreeding becomes insignificant as either homologous recombination or natural selection take care of the problem.
