Idk about doctor's protocol, but I'm very familiar with ems protocol for women.
Ask if there's abdominal pain. If so, ask when their last period was. If never, you might need to do a pregnancy test to rule out ectopic pregnancy. Do a couple scans and drop the bad news. They lack ovaries, fallopian tubes, etc. Charge their insurance for all of this. Rinse and repeat.

Nevermind about the insurance part. Make the government pay every time these mentally ill people demand to be treated as a woman, medically. Full scans, pap smears, pregnancy tests, etc.
Medically, grind it into their head that they are lacking the correct organs a woman should have, all the while ensuring a steady stream of job security by doing all of these frivolous tests.
Tl;dr treat as a female, including all the tests you already know the outcome of.
