At EA for example, a couple of people are in charge of creating the vision for the games. So it all boils down to the personal preference of a couple of individuals. When you see gameplay patterns repeating themselves in games, it's because some people at the top like it. These people have been in the industry for decades. Some of them are 50+ years old. This is why you don't see that many revolutionary changes in games. When they retire, you'll see some change. But until they do, you will see the same old and tired formulas. The same applies to Ubisoft games. The bigger the company, the more this is true.

They introduced graphic violence in Soldier of Fortune on PC back in 2000, and no one has touched it since then. Very few games have damage modeling, wounding, blood decals. There has been a radical increase in cursing in video games in the last 15 years or so. There is nothing in BF1 or CODs multiplayer experience that would warrant an "M" rating. At most, you see a brief bloodspray hit indicator. Every mainstream shooter in the 1990s was incredibly violent compared to what they have now.

Violence is completely acceptable in today's society. Aside from Linda Hamilton's side boob and the 2 or 3 uses of the f-word, you could play the original Terminator movie unedited on television.
