It's not about inability to afford it by any means, we could have the greatest bullet trains and high speed rail the world has ever seen tracked on some of the most beautiful lands on this planet, but frankly, the US was built on a few core things, one of those being freedom - something one does not find on public transportation, Nigel. Americans can afford healthy food. The ones that don't eat it are the ones that don't care about eating healthy and just want something that tastes good. I can literally walk a block from my house in the suburbs to the local grocery store that is packed full of cheap fresh meat and produce.
