I was just in the ER the other day and told them to fuck off when they tried to give me painkillers. If you're dying in excruciating pain then ok take it to make your transition easier like how they used morphine in Nam, but a lot of the people taking those pills know exactly what they're doing. I do think that it should be mandatory to warn people about the possibility of addiction before giving them certain pills though.
