"Krokodil" is still deadly for the reasons I mentioned, pure desomorphine isn't really. Most of the damage to a junkies health isn't from the opioid itself, they're actually not harmful to the body directly at all. The damage is caused by contaminants in the drug, infections, poor diet, dry mouth causing tooth decay, injection complications like scarring or lymphatic damage from missed shots or caustic substances injected under the skin when the vein is missed and brain damage from overdose and respiratory arrest. The reason fentanyl kills people is because of it's potency. A massive overdose and reside in 5 milligrams of powder, if that is injected and the person is on their own, they pass out and stop breathing. There's no actual toxicity, which is different to something like meth, which causes brain damage, it's neurotoxic on its own

The agent used has a very high vapour pressure, it isn't volatile like sarin, thus it has to come into direct contact to work. This makes it a very shitty weapon of mass destruction, but ideal of assassination. Even if it was as volatile, you still would need a huge quantity to kill lots of people. Look at that cult in Japan that used Sarin twice, Aum Shinrikyo. The first was the Matsumoto sarin attack, they used 12 litres of the stuff AND had a machine to vapourise and release it and only 8 died. The Tokyo subway attack was a confined space and they used multiple sarin devices and they still only killed 12.
