Several theories so far:
Pre launch sabotage. drill the hole, cover it up with an insulation panel or disguise with plastic/plaster, knowing it will fail in orbit.
Post launch sabotage, someone on board did it, possible while docked at the other space station
Russian cosmonaut lost it and attempted bizarre suicide while in the toilet.
