Only the most optimistic polls ever gave the positio of largest party to SD. The kind that is just a step avove unskewed polls in methodology. Reliable ones hovered around 20% and that is pretty much what happened.
Any systematical issues are baked in Swedish election system and its lack of privacy and impartiality. In Finland the parties don't campaign on the last day but in Sweden they crowd the polling stations and even handle the election tickets.

very simple, when they count the votes they just pour them on a table and sort them by party. take half (or more) of your SD pile(and count them) and throw them, replace with as many S votes as you just threw away. its bulletproof and our system is a tragedy. and anyone can count votes if they want to.
