No. After the incision is made the implant is put in. They make room in the breast tissue initially but the skin adapts overtime. When mothers produce milk their breasts expand naturally so the implants fit accordingly.

The implant is set under the fat and mammory glands, it can cause damage but most succesful ops do not affect the milk production. It can, however, cause pressure on the implants from the breast tissue swelling (forming milk) and burst the scar open or even the implant if it is of low quality.
