user, I am in the same boat, although improving at it. It's all about the reward circuits in the brain (muh dopamine etc.).
Technology and our parents' lack of strict disciplining has fucked us up. I had ZERO willpower when I was 18, but I built it up somewhat, and I'm still doing that.

Set up some rules for yourself to get rid of the dangerous drug known as dopamine. Basically, you'll be looking to increase your sensitivity to dopamine again by cutting out easy access to it via porn, sugar, video gaymz etc.

So yeah, cut out:
>porn (big one)
>sugar (big one)
>video gaymz
>social media like facebook, snapchat (I really, really hope you're not such normie tier scum, but going to list it anyways)
>any kind of shit food besides sugary food

You'll most likely fail if you try to cut these out all at once. It's because you'll feel madly deprived of dopamine and it'll crush you. Cut out something like porn first (you can still fap using your imagination), then after a week also cut out sugar etc. etc.

You will then get "high" off of the little dopamine you get when you do your chores/work because it will be the only dopamine you will be able to get.

Also something to add - not having deep and meaningful relationships with family members, friends and/or a romantic partner really fucks this up too. Our natural state is to have this and you're much more capable emotionally when you have this box ticked. Unfortunately, the modern age has fucked this up for us too. Simply a consequence of our time (social media making us less social, society (and our parents as a result) not understanding the value of being close).

So look to develop some deep bonds with people you trust over a long period of time. I am still lacking this, and have lacked this for a long time, but I'm generally strong enough to handle it without going mentally ill.

We have trouble doing things repeatedly or continuously because of an "inner rebel", a contrarian force that is triggered when we try to give ourselves an order. Being a Jew (but a Nazi Jew) I have figured out a way to deceive this inner resistance to discipline.

Here's what you do to finally become effective and disciplined.
Instead of telling yourself "I must read/program/train/whatever for 5 hours every day" (which triggers your inner nigger to refuse or sabotage this work schedule,) you take the opposite approach.
You're allowed 2 1-hour work blocks per day and no more. You set the alarm, and once it goes off you don't get to work any more even if you feel you're not done. The magic of this is that your inner nigger will now work in the opposite direction, and for the rest of the day you'll now find yourself keenly thinking about the next 2 hours and how to spend them efficiently. You might sneak in some extra work here and there when the desire becomes just overwhelming.
See how that works? Now instead of half-assing a 5-hour routine for a week and quitting, you'll be putting in 2 hours of pure focus, plus a lot of auxiliary time thinking about your work instead of how you used to cast it aside and purge your mind of it formerly.
Humans are stubborn When we force tasks upon ourselves, our impulses undermine us. When we force limits upon our work, the same stubbornness works in the opposite direction.
That's my system for overcoming procrastination, inconsistency and a (seeming) lack of will.

Depression. Start exercising for 20 minutes a day. Spend at least half an hour outside everyday. Take a multivitamin and fish oil as well. Cut down on added sugars and get your fix from blueberries or other anti inflammatory foods. You should bounce back to normal in a few weeks with minimal effort.
