It means she was either granted a clearance before and never told her security administration officer to report it after it happened or it was missed during the investigation.

Polygraph tests are not 100% accurate. Some people can lie through their teeth and never have the polygraph reflect any change in emotional state. There is a reason why polygraph results are not admissible in court.
