I got a bill for an appendectomy for $16,000 some twenty five years ago. After "insurance", it was still $6000. The hospital settled for $3200 of my money, which is what I suspect it actually cost, since I spent less than 24 hours in hospital.

Expensive medicine is not actually that big of a deal. Most of the money is well placed assuming free market incentives are actually present. But they arent.
Its the unbelievable hospital costs which are typically absorbed by insurance companies which get your tax money via medicare to offset their own fucks given. A gaping hole in the incentives created by free government dole.
If you just chose not to pay your medicine bills (and there are laws which work to protect you), or you eliminate tax involvement in medicine and insurance, assuming laws force doctors to treat people in emergency, they would be forced to lower the price of treatment, or increase prices to non-emergency (cosmetic surgery) options. Since the insurance companies pay the bills for the most part, the massive prices exist to rip off the insurance companies, who dont lose that much because they are invested 100% in hospitals and pharmacology corps and as i said earlier get your money anyways via tax.
The incentives are 100% to make doctors, insurance and pharmacy ventures rich as fuck and prevent you from refusing bad prices because your life is on the line and you have less choice. It is somewhat realistic and harsh but its bloated to all hell.

Hospitals raise prices because they know the insurance companies are going to negotiate down by a significant margin. If hospitals lower prices, insurance companies negotiate lower to a point where the hospital can no longer function. Were you charged for the actual medical costs, and insurance companies were removed from the equation, it would be significantly less, but you would end up paying more out of pocket.
