Certain kinds of services and events simply cannot happen in smaller towns and cities because of the population levels required for them to be viable. In most cases the voting population is way better educated and at the same time insulated from a lot of the issue that midwest and rural conservatives face. It's easy to be liberal on social views when you live in a city and don't see changes as a threat to your small town way of life.
When a job goes away in a city, you get another job in the city. When a factory closes in a rural area it can decimate not only the work force but entire generations of neighbors and families.
Farming area views firearms like a tool, suburbs tend to see them as weapons. Social issues are more liberal in a city being you bump into and interact with a wider range of people. Religious has less of a hold on city areas too, so more pro choice.
Small town USA people learn to count on neighbors over gov, suburbs and city folk tend to not even know their neighbors. So of course rural america looks at gov and thinks how they don't even need them and city folks don't understand that view point being the city falls apart without the gov.

Cities tend to be liberal because 1) the majority of citizens are liberal, and 2) the majority of citizens live in cities. It's not hard to figure out.
