It's probably going to be a mix, cybernetics won't be used as much as in cyberpunk, most thinks will happen externally, but those guys who will get a brain implant will be super-specialists. That brain implant will probably be implemented to be flexible and based upon configurating nanomachines within the brain itself. They still have to update those implants every 4 years but the benefit they will get from it, will make it worth it. It won't be commonly used though, only the most elite of the elite will ahve to use it at first.
They try to sell transhumanism as evolution but part of the reason the npc meme rustles so many jimmes is because it's so accurate, creating npcs is the ultimate goal of transhumanism.
They want us to get microchipped, carry tracking devices, put more drugs in the water supply, download our programs from the mainstream media (anything else is a thought crime), trust ai, give up our genders and become androgynous, and finally take away our ability (or will) to reproduce. As it stands, the human brain isn't very user modifiable, and is relatively inefficient, so it is more likely that a superintelligent AI will emerge before sufficiently advanced neuroengineering can be developed. It's a lot easier to build a 747 than it is to start with a bird, gradually replace its body with mechanical parts, and slowly transition it into a robotic bird the size and speed of a 747, all of this without killing or significantly harming the bird.

If anything, NPCs are probably going to eventually go the way of the horse. The reason NPCs exist is for use as a workforce in order to keep the PC elite in power. Once NPCs outlive their usefulness and everything becomes automated, they will likely be killed off.
