The truth is Cuba sees their doctors as "property" and the nature of our deal with them reflects this. We're not bringing in individuals and paying them salaries, we're bringing in human machinery and paying Cuba for rent.
Some lefties correctly pointed out that outsourcing is a similar business relationship. They aren't entirely wrong, but our deal with Cuba is worse because Cuba is a communist dictatorship. An individual in an outsourced company in Brazil is still protected by our labour legislation; his family isn't being held hostage. And competition in the outsourcing space means the gap between employee salary and what's actually charged from clients tends to be smaller than whatever is going on with Cubans.
