Hey guys, so I kinda have nobody to ask advice for so I guess you're the best I have at the moment. Also I'm pretty high so this might be a long read. So there's this problem that's been fucking me up real bad recently. Some backstory which I think might be connected with it.

I kinda ignored the problem but after some time I noticed I can't piss even when I'm at home when I have people at my place.
>So the problem progressed to the point where I'm now having trouble pissing in general, my kidneys hurt(like up to 8 on pain scale), my piss is like almost brown also my insides feel edemic often and I feel like really drained and winded alot of the time. I'm not sure but I kinda think I might be getting...depressed (I kinda don't like throwing around terms like depression lightly so I'm not really ready to say I am yet.).

I mean ok since I studied a little bit of medicine I decided ask some more questions about what he thinks about pathogenesis in my case, what are the root problems and he kinda didn't really answer to any of them he just kinda said it's like...idiopatic. which is a cheap answer. He also did a urinalysis which showed nothing but a couple of days later I realised I panicked and downed a liter of water to make me piss which probably diluted my piss so much that the analyser gave all NEGs.

Are you by any chance also an alcoholic? Brown piss is usually a liver thing, and abdominal edema plus shortwindedness could be ascites (fluid in abdomen because of portal hypertension).

But you would have to have either cirrhosis or some type of hepatitis, neither of which is common, especially in young people, unless you're a serious drunk, an IV drug user, or an incautious philanderer with Southeast Asians.

Is it possible that you could have hep or alcoholic liver disease? If so, go back to a doctor and say you're worried about that.

If not, dunno. Prostatitis seems possible, but I assume you'd need to get an ultrasound alongside the labs.

Go ask for an ultrasound.

If you think you're seriously in danger, and from the sound of it you think you are, you should consider the emergency room. If you just don't like the doc's diagnosis, go see someone else and get a second opinion.

Thing is, your UA was negative. Drinking a liter of water probably isn't going to change it that much that fast. If it was rhabdo or blood in the urine, those aren't likely to come up negative. Possibly same for kidney stones (even though you're only 21, I think that's a distinct possibility here).

Honestly, part of your story—"I can't pee at home anymore and it's been like this for a few years"—suggests there's a psychiatric component, and a negative UA plus no red flag signs or symptoms would probably lead me to believe there was a psychiatric issue involved. At the very least, you've effectively told the doc that it's probably not going to kill you in the next few weeks given you've lived with whatever problem you've had so far for so long.

Again, if you think you're in danger, you go to the emergency room. I will say it kinda sounds like a stone to me... and usually resolve themselves.

I'm not a drinker, I didn't like the way alcohol feels, just gad a few parties ever from like 17 to 19 until I started smoking weed and ditched it completely. I didn't drink since 19 at all, only had a bit on the new year but still was sober enough to be appointed puke nurse to my friends who had too much. In the last couple years I had a few trips of acid (like...5 acid trips 2 of which were NBome I think, 1 mushroom trip, an mdma trip (I kinda have a fascination with drugs which is why I study biochemistry) , a bunch of weed (I smoke like 3 days a week usually, other days I give it a rest.) and like some cigs, nothing too crazy and I'm doing my best to lay off and quit. So...do you think that much would really damage the liver of an average man? Also bilirubin in urinalysis was negative so liver should be fine?

Yeah, sounds like your liver wouldn't be in trouble.

You mentioned slamming down water before your urinalysis and you peed fine. Are you adequately hydrating yourself with water?

Also a regular blood analysis in two days. Also please advice if I should I ask my doc for xanax (which the urologist said I might wanna try) just to rule out psychological reasons if they had any hand in this whole mess and maybe offset depressive state of mind or knowing I might be prone to drugs knowing for example weed habit and kinda scared I might get addicted and shit should I don't even ask for them. I think my family doc might prescribe it (Im not entirely sure) or would give me a referal to a psychiatrist which I would just ask the psychiatrist for meds and ditch him because psychologists here don't really give a shit and I'm you guys are like most I can go with in talking about this whole thing.

Also...What do you think is the chance it's just a stone that the urologist missed? I'm kind of losing my trust in doctors (which might be influenced by my studies). Would it still be possible that I'm just (I know it's supposed to hurt like shit) piss the fucker out and be completely fine afterwards? Should I even consider cancer (which should be unlikely since my age and no family history?)

Yeah I drink almost like 2 liters a day and it still looks pretty damn dark

Sonography isn’t 100%. The diagnostic for stones is to basically piss through a filter screen and look for stones. They may or may not come out when you piss for the UA.

In particular, if you don’t have a kidney infection, any stones are gonna get passed, or be so huge they’ll be detectable. The kind of stones you get from kidney infections are more problematic.

Okay so do you the that 1 month of feeling good would be caused by the 10 days of ciprofloxacin 500 mg. Cause I kinda think I might have stones, they are rubbing in my kidneys
(pain) /urethra(which caused urestasis it started an infection, then the antibiotic kinda extiguished it but I stopped using the antibiotic and then it's just kinda came back since the stone is still there causing shit which is kinda why I feel so shitty now?

You could also just have a bad UTI. That you have a history of holding your urine is a risk factor for developing UTIs.

UTIs are not exclusively attained via sex. It's just bacteria getting in your dick, man. Lack of urination can cause a UTI because you're not excreting whatever's in your urethra.

Your piss shyness can cause a UTI, like says. That you bathe properly is pretty meaningless. A chronic UTI could easily go up the ureters into the kidneys and from there cause a bad type of kidney stone.

“When you hear hoofbeats, think of horses not zebras.” UTIs are extremely common and fit most of your symptoms. If it’s chronic (how long have you had the serious piss problem?) it might be something that a short dose of Cipro wouldn’t completely fix.

Not necessarily. The sensitivity of leukocytes in UA is around 75%, so one in four UTI cases won’t be positive leukocytes.

Also do you think the "shy bladder" was a symptom or a cause. Cause I never really cared for it, I used urinals, pissed with guys while fishing in the nature and other outdoors stuff, never been like...traumatized or insulted or stuff don't think it's unnatural it just sort of appeared. I don’t know what the sensitivity is but I imagine it’s lower than leukocytes in the UA.

Did they check nitrite levels in your piss?

Honestly I would imagine the probability of the two tests is linked; that positive blood is highly sensitive for positive leuks on the UA but nonspecific, rather than the sensitivity of the two being independent.

Just kinda a crapshoot guess I learned in uni during hematology. Higher CRP would suggest a chronic (mostly) inflammation presence in the body.

I also kinda have this thought in back of my mind that I might just be fine without doing anything but idk, maybe ask for some xanax and just ignore it but I'm not sure I'm ready to do that until I know I'm safe. you already have your answer. Look at how this started, the things you associated with being the reason you couldn't urinate. look at how it has progressed. every doctor you go to, you assume that they are missing something, they're wrong and your own opinion is the one that's correct. when the doctor's prescription does not match your expectation your symptoms worsen, but when the doctor says what you want to hear, miraculously you feel better. extreme urine retention can turn the urine a dark shade of yellow or orange. The lining of the bladder will dissolve into the urine. happens all the time with long-term catheterization patients.

you need to talk to a psychologist and a psychiatrist and probably a couple other specialists. stop assuming you know more because you Googled what the big words on the lab report were.

Before all this during the 5 years I lived alone I've had 2 docs appointments (one of them was was just to get like a paper for my drivers license, other was like to get out of class or something) so it's not like I have a history of looking for nonexistant problems. I never asked or wanted this.
I would gladly take the benzos and would love to just sweep this whole thing under the rug I just gotta make sure my kidneys are not in any actual danger. I really don't want to need to get dialysis multiple times a day and wait for a transplant at 21.

Id suggest to drink a lot more water. Attempt to get to 100oz a day and youll probably be pissing fine.

Kidneys can safely process a liter of eater an hour in case you’re wondering how much is too much.

thanks man Im tryina do that, I don't think my hydration habbits were that bad since I used to hit the gym and had my regime of slowly hydrating myself with like 2 liters a day, or 3 of I hit the gym.

Anyway you guys think I should maybe force myself out to gym or jogging to keep metabolism up or nah?

It's not a nonexistent problem bro. It's just probably not being caused by a structural problem in your renal or urinary tracts. The mind and body inhabit the same space, and your anxious mentation will affect your physical health.
