I liked Naples. It felt very authentic, and the people there were very passionate and enthusiastic about....everything. I truly felt like I was in Italy. There were hardly any tourists because of its reputation. I totally knew why people avoided it. First, before going into the city we were pulled over in our rental car because apparently it wasn't allowed...but really they just wanted bribes. We couldn't speak Italian so we just acted really stupid and they got frustrated and let us go. Next, some guy on a scooter snatched my phone out of my hand, but luckily he dropped it. There was trash piled up everywhere. Traffic was India-tier. I tried to do a Grindr hookup and I walked to this guy's commieblock (?) and the doors weren't numbered. I asked the neighbors and they started yelling his name. When I got to his door, his wife answered and I saw him yelling in the background with a spaghetti stained wifebeater. His name was Luigi lol. Unfortunately, couldn't get dick that day because I realized I was in the slums...and there weren't any niggers but there were a lot of sad, angry Italians just staring at me. It was truly terrifying. I saw Mt. Vesuvius in the distance. It had a weird shape to it. Very cyberpunk, very bizarre city.
Pizza was so delicious, I still can't stop thinking about it. The alleyways were very narrow and sketchy. The metro stations were really cool with their modern art and graffiti.

It was dingy and scary but fuck I loved it so much.

Sardinia is a more tourist place and very expensive as well (unless you go there in spring or autumn) and universities there are just average. Biggest campuses for Americans are in Florence and Rome but they are essentially US 2.0, it would be like being in the US but with some Italian features (well, if you like that go for it obviously).
Assuming you are a guy, you could try for some seasonal work like a male au-pair if they exist (a friend of mine is in California now being an au-pair) or something related to summer jobs.
If it is solely to study, then look also at the organisations US-Italy that have a few programs to come here and study.

A friend of mine took a bike trip with his girlfriend through germany, crossed to switzerland and then to italy. He said the trip was lovely, until they got to italy. Apparently italians don't like bicycles and many were shouting at them, insulting them and shoving them aside. Then when they tried to go back to germany they tried to get on a train but the trains didn't allow bicycles on them so it was a nightmare to get out.

I've never been to italy but I guess I'll visit some day... without a bicycle.
