Because the USSR was no longer around and the west sanctioned them they had nobody to trade with. The US was content to just sit around and let them starve, even programming the public to accept this by pushing a huge media campaign about how north korea is going to colapse any minute. To speed it along they had CIA operatives offer to trade bags of rice for copper wiring to encourage the north koreans to dismantle their industry to survive.
It didnt work however, they recovered, now they have nukes. Pyongyang is the most developed part of the country because it's the capital just like anywhere else in the world, but that doesn't mean the rest are left in poverty
