We've imported shitty quality scum from the third world in huge numbers while prohibiting ERs from turning them away. The result is the ER has become a free general clinic for the third world. Since they can't or won't pay, the cost gets passed along to those who do pay.
As someone who works in the lab at a hospital, lab work is absolutely insanely priced. However, there is a reason for it. We run a test called a BMP, a Basic Metabolic Panel. Basically it will test simple things like potassium level, sodium level, Chloride, etc. This one test costs upwards of $50 and it takes us 20 minutes to run it. For patients that come in from the emergency department, they have STAT tests, which are prioritized first. There is a stat fee of $25. One test can run a trauma patient $75, and the ED usually orders a battery of 15 tests standard. However, the reason they're so expensive is because like anons said, there is a lot of insurance fraud. We also have problems where homeless people will come into the emergency department, tell them they're sick so they can have a place to stay out of the cold. We can't just turn them away, so they usually will run a test or 2, tell them they're fine and then kick them out. That's $100 worth of tests that we won't get paid for simply because some homeless dude wanted a place to stay for the night. It's true there is a huge monopoly on medical supplies, which is another big factor as well. The collection tubes alone for the test can cost $15-$20 depending on which collection tube needs to be used.

It's crazy that people still trust our corrupt government. You know all of the companies making bank off of medical supplies are owned by someone who has friends in some position. The entire thing is a who knows who to make money off of the general public.

Proper regulation. With every regulation and law there are people that find loopholes and manipulations, that's capitalism at work. As long as systems are in place there will be people trying to profit. That can't be helped. I'm saying that the regulations need to be a constant work in progress.

Apart from the factors already mentioned itt, when populations become wealthier they dedicate a higher % of their income to health.
This is simply because healthcare beyond the point of emergencies is secondary to other needs. Thus why when populations become wealthier, you start seeing them spend more into these kind of secondary costs. In america the costs are high compared to other countries mostly because of preemptive medicine, aka, the one you buy when you don't know what the fuck to do with all that money anymore.

I'd say insurance companies are even bigger culprit for high medical costs in US. They are trying to make profit out of everything by all means they can. Too many lawyers involved in things they shouldn't be involved in at all.
