I honestly don’t think you can keep an 85 year old alive that long on a vent. By this time they have had to do a trach on her and that prolongs recovery for a while because afterwards you have to not only wean her from the vent you have to wean her from and close the trach.
Consider this: at this. Moment it is considered UNETHICAL to provide “continuing care” for reasons other than “the benefit of the patient”. What they are doing atm is gonna get somebody’s license to practice pulled, if not several somebodies.

Depends on your illness but it's pretty amazing how long we can cheat death with our modern treatments.
Intensive care creates a lot of problems and complications on its own but they are manageable if you want to just keep a person alive technically.
By that i mean that the person still has a beating heart. Nothing more. I'm not talking about being responsive or anything else.
We just need to make sure that you get enough nutrients, oxygen and maintain a reasonable blood pressure and make sure toxins and waste are evacuated from your body.
For that we got ventilators, infusions, vasopressors and dialysis machines.
If you have a brain injury and are comatose it is possible to keep you going like this for years.
Even people with diseases like cancer can be kept alive for quite some time but most of the time the disease determines the lenghth of survival and is the limiting factor.
Intensive care related complications like pneumonia, neuropathy and stuff are further limitations.
When talking about being alive i mean zero quality of life just being technically alive.
We do that a lot, actually. I have a lot of patients that are coming to my ward where i know they wont make it but either the patient told us before to keep going or his relatives refuse to accept reality and want us to keep going.
We could refuse and stop therapy when there is no chance for a cure or improvment of the condition but nobody wants to risk a law suit so unless all the relatives are on board we continue tending to our vegetables.

Technically there isn't. Usually justices have the good grace to retire when they can't work any more or they have something like a heart attack where they just drop dead before they go back to the bench for the next case.

When I was a surgical intern we had to run a 15 bed ICU for two months. You couldn’t get a senior resident to even come into the room. I am sure things have gotten better in the interval but if you had suggested keeping an 85 year old alive for months on a vent everyone would laugh at you. The risk of pneumonia, mucus plugs, pneumonia, etc. eventually accumulate and your fucked. Maybe you can do better now but there has to be an upper limit. As far as being extubatable IMHO she was marginal at best before the did a lobectomy. Personally, I don’t believe there is a chance of weaning her.

In my experience in almost a decade of neurosurgery in those cases where the patient is truly brain dead after a couple days you can’t keep their blood pressure up even with an Epi drip so I wouldn’t crush my soul over it. I found it impossible to keep a truly brain dead patient alive for more than 72 hours.

But lung surgery at 85 years is a tough game.
Important thing for her is to breath on her own with no assistance. That is hard to achieve depending on how much lung tissue has been removed. Pretty likely she is trached by now. Anyways, she is just on pneumonia away from hypoxia and brain damage.
But even then she can still be kept alive.

I've kept a 95 year old woman alive for 6 days after lying in her kitchen for 3 days with a complete occlusion of her left internal carotic artery turning her entire left brain into spaghetti, before pulling the plug. We knew it would end this way and could have let her go the moment we found her but she hasnt written a will, couldnt talk at the moment and her daughter crying and under shock told us to "please dont let my mother die". So we did.
I told her that it was of no use after seeing the brain scan but proceeded anyway just to avoid a possible lawsuit.

He was “doll’s eyes” level with no spontaneous respiration for 4 months. I gave him a ride home the day he was discharged. The other was a kid with a motorcycle accident grade 4 concussion. At 6 weeks he had no spontaneous respiration effort and we were trying to place him when he just woke up and started talking to mom.

It's amazing what young people can come back from. I once had a 24 year old biker with open head trauma, pneumothorax on both sides, lacerated spleen and liver and multiple bone fractures. Guy spent 7 months in the hospital, 3 of that on my icu 1 under sedation and being ventilated. Had pneumonia and pulmonary emboly and what not.
Guy is playing football again today. Not as good as he used to but he keeps going.

Actually getting slammed in court and losing your loicense is still very rare and you have to fuck up bigly. Most lawsuits dont lead anywhere or get mediated by the board of doctors but it's still an enormous pain in the butt. Lawyers are among the worst people on this planet.
Medicine is stressful enough as it is and we work under a lot of pressure, a lot of it being of an economic nature so we tend to avoid additional stress. If that means turning your beloved grandma in a vegetable, so be it.

To be honest i have never treated a severe burn victim (in a hospital, outside driving ambulance yes) so i dont know. And i've never seen Stephen Johnson's yet. But i imagine the problems are pretty similar given that most of your skin is lacking. So hypothermia, fluid loss, infections etc...

I think you use “clinic” for inpatients where we would use the word “ward” to describe different parts of the hospital subdivided by location and specialty.
You have to get good if you want to survive as a resident. Otherwise you get a rep as a pushover and you get buried. Funny since once you go into practice you tend to whore up everything you can get your hands on.
We use hospital (Krankenhaus) and clinic (Klinik) synonymously.
Outpatient facilities are called "Ambulanz" when in a hospital or "Praxis" when it's a non hospital doctors office.
Inside hospitals the different departments are also called clinics but commonly referred to as departments.
Wards are your actual units where your beds are.
And it is also funny because we retain that use of “clinics” when you go from 2nd year to 3rd in Med School. It is called the “Clinical Years” and is referred to as “going to the clinics” but that is literally the ONLY time you will hear it used that way.

Back in the day when all of the doctors did NOT work for the hospital but had “priveledges” (which could be pulled by your peers) so there were NO doctor’s offices in hospitals. So before the 80s very few hospitals had any part of the hospital dedicated to offices for anybody other than hospital administration. So “ward” is still synonymous here even though most hospitals now employee great numbers of doctors and many of them have offices in the building (or a new adjacent Doctor’s CLinic for outpatients near the hospital (like across the street).
