They didn't vote for him. The current president (Pedro Sánchez) got the presidency after a motion of no confidence at the Spanish parliament. The previous president, Mariano Rajoy, was forced to step down and the new one, Sánchez, replaced him as he was the main leader of the opposition.
What happens with Pedro Sánchez is that since he became president he has not announced new elections, and as a result, Spaniards are angry with him.
Sánchez's government aims to last out its term, so there are no plans for incoming elections, which sounds pretty undemocratic to me since the guy is president for a matter of institutional luck.

It was great fun. Massive demo, lots of camaraderie, and then we went and ate squid sandwiches at the Plaza Mayor, patatas bravas at the Cafe Abuela, and garlic prawns at Cafe Abuelo. I went there on the Vox bus, and we pulled in at Casa Pepe on the way home to buy assorted Francoist souvenirs. The Popular Party bus came there too, and despite all the bullshit they call Vox, they were all buying the same trinkets with Falangist symbols and bottles of wine and oil with the face of Franco...
If these people are "extreme right", then I can only assume this term means "mostly middle-aged couples who are sick of bullshit anti-national policies and who have a romantic nostalgia for their youth, and use political activism as an excuse to go on a fun day trip".

I saw only one Falangist flag on the square. Our group had lots of people with the old Francoist flag on their facebook profiles and on stickers on their mobile phone cases and so on, but we all just took the national flag to carry. It was a conscious attempt to show solidarity beyond narrow party lines. As well as Vox, there were coaches from our region taking supporters of the assorted "mainstream right" too.

But the Jews are powerful, so if this is a price to pay for the good stuff Vox offers (as with Bolsonaro in Brazil), it's a very small one.
