It's not easy, but I am in the process of doing exactly that through very slow steps every day. Every day build my discipline piece by piece and just try to be consistent and begin to derive pleasure from completing steps of my long term goals. It's important not to let your guard down and let old bad habits sneak back in when you've been doing good and think you deserve a reward.
