Your sister doesn't need to go away to Oxford to become a nurse. She needs to remain a major part of the child's life. You can facilitate this and take up the slack when she's not available, you can live with her and feed her and take her to school everyday and pick her up, but her mother must help raise her in some measure. I don't like the wording of this OP where you are apparently raising her instead. Raise the girl alongside her mother, as you would if you were the father or the child's grandparents.
