It's not a speciality form. It's a standard fill in the blanks immunization form with a signature and doctor's number being included. An office stamp is also another way of verifying it. Many schools have immunization forms that they require students to fill out and submit or they won't even be allowed to register for classes. Any signature verifying records like immunization or health factors pertaining to academia reasons should be free. Change my mind.
