The US cares for their military needs through NATO, and defends their shipping/freight/trade through the US Navy and US Airforce. The US cannot afford healthcare AND afford to defend global markets and modern civilization. Everyone in power knows this, they just pretend to act like idiots for the media to keep people from understanding.

It's logistics and it's always been logistics, when we say america is a 'big' country it is meant both literally and figuratively. Our 10 largest states are as big as the entire eurozone (ignoring alaska or bring that number to 4 largest) and they aren't even geographically next to each other.
Trains are impractical, as are most other forms of transportation other than car and plane.
Ignoring alaska our population density is about 100 people / sq mi vs 300+ for the eurozone (worse if you ignore finland), but that assumes even population density. of which 10% of our population is contained in the 10 major cities only covering a few thousand square miles.
There just isn't critical mass in 90% of the country to support large centralized services, that leads to inefficiency and duplicity of costs and capabilities.
Sure the metro areas could sustain something like that but a federal government mandated system is simply unaffordable, if health care is a right, you have to provide it to everyone no matter where they live.
It's not a matter of would or wouldn't work the same, it can't work the same.
