I think that's often right. A lot of people are just too immature and have never learned how to use force when appropriate without losing control. What happened to your friend was awful.
I think in the past boys learned to use force appropriately from having normal fist-fights or tussles with other boys followed by making up. They learned the relationship between force and pain and how to fight without losing it and really hurting the other person.
